What this document does

What we'll ask you

Plain-English questions, one at a time. No legal knowledge needed — if you don't know an answer exactly, your best estimate is fine.

1. Let's make sure your pet is looked after if something ever happens to you. This directive records who you want to care for your pet if you die or can no longer care for them, their routine, and any money you want set aside. To start, what is your full name (the pet's owner)?
2. Tell us about your pet (or pets). For each one: name, the kind of animal and breed, and rough age.
3. Do you have microchip or registration numbers for any of them? You can list them here, or skip this.optional
4. Does any pet have medical needs, medication or a health condition a carer must know about? Include doses and timing if you can. Skip if none.optional
5. Describe their everyday routine — feeding (food and times), exercise, where they sleep, and anything that keeps them settled.
6. Who is your vet, and any other regular carer (groomer, boarding kennel, dog walker)? A name and phone number is ideal.optional
7. Who do you want to be your pet's main carer if you die or can no longer look after them? Their name and your relationship to them.
8. Has that person agreed to take your pet?
9. Who should care for your pet if your first choice cannot? A backup carer, with their name and your relationship to them.optional
10. Is there an amount of money you would like to provide to help with your pet's care? You can give a figure, or skip this. (Note: to be binding, a gift of money for your pet's care must be set out in your will — this directive records your wish, and we will say that.)optional
11. Any special wishes for your pet's care? For example, keeping siblings together, no re-homing to strangers, or end-of-life preferences.optional
12. Your state or territory — so the letter is dated in your time zone.
13. Your name and email — so the draft lands in your dashboard and inbox.
